United Airlines Mileage Plus Elite Status Match Program
The Mileage Plus Elite Status Match Program* will match the elite status you hold with a participating competitive airline, up to Premier Executive (1P). This program is available to new or existing members, provided you have not previously received a status match.
Just print, complete and fax this form, along with documentation showing your elite status on another airline carrier to the Mileage Plus Customer Service team at 1-605-343-4104.

*Rules and Restrictions may apply. You will receive elite matched status in Mileage Plus for a 90 day period, or
thru December 31, 2008 whichever is sooner. During this 90 day period, you must complete a specific amount of paid qualifying miles on United, United Express or Ted flights to retain the elevated status.
The specific amount of paid qualifying miles or segments are as follows:
Premier/2P = 7,500 Flight Miles OR 10 Flight Segments
Premier Executive/1P = 15,000 Flight Miles OR 18 Flight Segments
